Home Care LINK's reports provide immediate confirmation following the visit of every in-home caregiver. Any authorized agent may access these cloud based reports using any computer

Alerts

During Setup each action and observation is prioritized as either normal or critical.

Critical activity scans are immediately sent to the Care Manager and whomever they designate.

Alerts can also be sent if a critical activity is NOT scanned.

The Caregiver may also send immediate messages using the “Send Now” button.

Reports

There are three standard reports.

Caregiver Chronological Report
Caregiver Electronic Visit Verification (EVV) Reports
End of Visit Observations Report

Reports may be filtered by date and time, caregiver, and several other variables.

Reports are immediately available following each caregiver visit on any device.

Alerts tell you immediately:

a. If a Caregiver doesn’t begin their visit.

b. If there is a significant health change.

c. If the Care Recipient has COVID-19 symptoms.

Examples:

a. As of 07/13/20 12:45 there is no record of “Claudia Johnson: Begin Visit” having been scanned since 07/13/20 12:30.

b. 07/13/20 14:19 Critical Note – Claudia has a new wound. See pictures.

c. 07/13/20 14:17 02. Seems “off”, confused, not themselves
07/13/20 14:17 04. Increased fever, see report

Caregiver Chronological Report

Details of each caregiver’s visit including:

Dates and times for each action or observation
Caregiver’s name
GPS for each record
Notes, if provided
Picture icon, if pictures recorded

Begin and End dates and times for each visit

Home Care LINK's Chronological Report records each caregivers' ADLs and observations. Each report shows the caregiver's name, date, time, and GPS location

Home Care LINK's End of Visit Observation Reports summarize any in-home changes that might impact a care recipient's medical condition. It can also provide alerts for critical change

End of Visit Observations Report

Records the caregiver’s observations of condition changes in the care recipient.

Conditions may be specified by the healthcare provider, the home care agency, or the family.

Report shows observations for every day within the selected report period.

Caregiver Electronic Visit Verification (EVV) Report

Shows total visit hours for every caregiver.

Shows the exact Begin and End date and time for each visit.

Shows if any caregiver failed to record a beginning or ending time for any visit.
